Available Positions

Duties and Responsibilities:

The work duties and responsibilities of this position include, but are not limited to, the following:

  • Understand the service definition, including exclusions, of the service provided.
  • Perform all services as scheduled within service definition.
  • Use teaching cues to help the consumer obtain specific goals outlined in the Provider Plan.
  • Enable the consumer to have choices in their daily schedule and activities.
  • Monitor health and safety.
  • Accompany and facilitate the consumer’s travel in the community when required as a part of the service.
  • Inform QP of consumer’s progress or lack thereof.
  • Perform personal care as needed. 
  • Lift and/or reposition consumers as needed.
  • Accurate and timely completion of daily documentation of services in Therap.
  • Proper completion of other required forms (i.e. weekly timesheets, incident reports, medication administration records, etc.)
  • Attend mandatory meetings and trainings as scheduled by the agency.
  • Assure consumer/family confidentiality.
  • Demonstrate respect for consumer and family property and privacy.
  • Report the need for protective services of a child/adult with disabilities due to suspected abuse, neglect, or exploitation.
  • If providing Residential services, abiding to the rules/standards outlined in the contractual agreement with the Agency
  • Other tasks and responsibilities may be added at the discretion of the agency

Training and Qualifications:

  • At least 18 years old
  • Must have a High School Diploma or GED.
  • Current CPR/First Aid Certification.
  • Maintain NCI or other acceptable alternative training.
  • Must successfully complete Core Competencies.
  • Must be able to read, write, understand and follow directions.
  • Satisfactory criminal record check.
  • Satisfactory Healthcare Registry check, OIG, sex offender and Google check.
  • Valid driver’s license, satisfactory driving record check and acceptable level of automobile liability insurance to provide own transportation to perform duties above.
  • Willingness to be flexible in schedule and consumers served as needed.
  • The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually.

Job Summary:

This position involves supporting individuals with developmental disabilities to participate in individualized day activities that reflect the consumer’s definition of a meaningful day in an integrated community setting with persons who are not disabled.  Community Networking is designed to offer the consumer the opportunity to develop meaningful community relationships and develop natural supports within integrated settings.  Services enable the individual to increase or maintain the capacity for independence and develop social roles valued by non-disabled members of the community. 

Job Summary:

This position involves supporting individuals, either one-on-one or in a group setting, with developmental disabilities to attain or maintain maximum functional level while participating in meaningful activities in a structured day program setting.  These services include a combination of training, personal assistance and supports as needed by the participant during activities, including ensuring safety in all settings. Opportunities provided for consumers include socialization, daily living skills, independent living skills, coping skills, decision making skills, relationship skills, as well as fine and gross motor skills.

This position involves supporting individuals with developmental disabilities in their own home to assist them with activities of daily living, household chores essential to the health and safety of the individual, budget management, attending appointments, monitoring health status and physical condition, and interpersonal and social skills building to enable the individual to live in a home in the community.

Job Summary:

The intent of this service is to meet the habilitation and personal care needs of adult consumers (18 years or older) who choose to live in an Alternative Family Living setting and receive support in a home-like environment.  Residential Supports is designed to provide flexibility and reflect the natural flow of the participant’s day. These services can be provided in an unlicensed home serving one consumer or a licensed home serving up to three consumers. The Innovation waiver uses the title habilitation technicians or direct care staff for this position.

Job Description:

Duties and Responsibilities: The work duties and responsibilities of the position of Qualified Professional (QP) include, but are not limited to, the following:

  • Supervise, train and evaluate assigned direct care staff working with individuals served by Reach who are enrolled with the Innovations Waiver program.
  • Evaluate the consumer’s needs.
  • Participate in the development of the Individualized Support Plan for all individuals assigned.
  • Develop habilitative goals with consumer and/or consumer’s family.
  • Monitor progress and revise goals with consumer and/or consumer’s family.
  • Provide family home visits to ensure program compliance and family/consumer satisfaction and/or schedule face-to-face visits with staff in all service locations.
  • Assure consumer/family confidentiality.
  • Monitor and submit all required documentation in a timely manner.
  • Report the need for protective services of a child/adult with disabilities due to suspected abuse, neglect, or exploitation.
  • Receive continuing education on issues pertaining to persons with disabilities.
  • Report the need for protective services of a child/adult with disabilities due to suspected abuse, neglect, or exploitation.
  • Attend Innovations waiver training classes and/or provider meetings as directed by the Reach President.
  • Monitoring and updating consumer files
  • Monitoring and preparing AFL privileging files for audits and compliance
  • Printing and calculating client authorizations
  • Other projects and responsibilities may be added at the discretion of the agency

All job requirements will be the sole responsibility of the employee to obtain and, if applicable, keep current.  Furthermore, any training and qualifications below, whether mandatory or preferred, will be at the employee’s own personal expense.

Training and Qualifications:

  • Graduation from a four year college or university, in a discipline related to developmental disabilities, with at least one year of supervised habilitative experience; or, graduation from a four year college or university in a human service field and two years of supervised habilitative experience working with individuals with developmental disabilities; or,  graduation from a four year college or university in a field other than one related to developmental disabilities and at least four years of full-time, post-bachelor’s degree accumulated MH/DD/SAS experience with the population served; or, a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing with four years of full-time accumulated experience in MH/DD/SA with the population served.
  • Knowledge of fundamental principles, techniques, and methods in developmental disabilities which include experience in the assessment and development of outcomes and activities.
  • Current CPR/First Aid Certification.
  • Current Blood Borne Pathogens/Universal precautions training.
  • Must successfully complete Core Competencies and required refresher training.
  • Satisfactory criminal record check.
  • Satisfactory Healthcare Registry check.
  • Valid North Carolina driver’s license, satisfactory driving record check and acceptable level of automobile liability insurance.
  • Provide your own transportation to perform any duties above.
  • Ability to be flexible.
  • The supervisor and the employee shall develop an individualized supervision plan upon hiring. The parties shall review the plan annually.

Additional Skills:

  • The QP is a critical position in the Reach organization.  The QP serves as a point of contact for both consumers and staff of the agency and is responsible for the documentation that ensures compliance with waiver requirements.  The position requires the ability to problem-solve, create and maintain schedules of supervision visits, and meet deadlines.  Excellent communication and writing skills are necessary.
  • The QP is expected to interact professionally with LME/MCO Care Coordinators, other community/agency representatives and members of each consumer’s treatment team.
  • A case load will be assigned to each QP, to include staff who are assigned to the Day Program, an AFL or consumer home, the community setting. QP staff may also be assigned to support the activities of the consumer’s Individual Support Plans, including documentation.  All staff employed by Reach must be supervised on a quarterly basis, with documentation provided on the Clinical Supervision Log.  Supervision includes both a training topic and performance evaluation as well as a review of the employee’s progress toward completing assigned Relias training and the personal learning goal.  Further, for those consumers served in an AFL or a family residence, the Clinical Supervision Log also includes review of safety features and requirements as described in the waiver documents.  That component of the supervision form must be completed on a monthly basis.
  • For consumers served by multiple staff and/or in multiple locations, the QP is expected to create a schedule such that the quarterly supervision is completed for all staff and includes all service locations
  • For the QP assigned to the development and assessment of short-range goals for each consumer, there are no supervision requirements nor home visits, but rather participation in the annual ISP meeting where the long-range goals are developed.  Documentation of the progress toward, or revision of, short-range goals are report on a quarterly basis, beginning with the implementation of each ISP.
  • Reach expects that the QPs will appreciate the importance of the staff support and evaluation component of their position.  The quality and effectiveness of staff is directly related to the superior services that the agency is committed to providing.  In turn, the QPs will be provided with training and support to perform their job at the highest level.
